Fields left blank on purpose
Four fields the forms require in your own hand.
This application prints nothing into four places, and that is deliberate.
On the letter of guarantee, the guarantor's name is marked 氏名(自筆)— written in one's own hand. It is a signature field. A printed name there is a common reason the document is handed back at the counter.
On part 3 of the application form, the declaration carries the applicant's or representative's signature, and the form's own note states that the date of preparation must also be written by the applicant themselves (自署).
On the last page of the questionnaire, the closing note asks the Japanese spouse to confirm what they have written and sign it themselves. The date beside it is typed; the signature is not.
The application form adds that if anything changes between preparing it and filing it, the applicant corrects the entry by hand and signs the correction — so take a pen to the counter rather than reprinting the bundle.
