The identity documents, and whose they are

Passport — the foreign spouse's. Copy the name character for character, including middle names, in the passport's own order (family name first). If it expires within about six months, renew it before applying; a period of stay cannot usefully outrun the passport it is stamped against. The passport itself is shown at the counter, not surrendered.

Identity photograph — the foreign spouse's, 40mm × 30mm, taken within three months, plain background, no hat, full face. It is affixed to the box at the top right of part 1. This is not the same thing as the snapshot photographs of the couple, which are evidence of the relationship and are submitted loose.

Residence card (在留カード) — the foreign spouse's, and only where they are already in Japan. Number, status, period and expiry are transcribed onto the form; the card is shown at the counter.

Family register (戸籍謄本) — the Japanese spouse's. It must be issued after the marriage was registered, because the whole point of submitting it is that it shows the marriage. A copy taken before registration proves nothing. Allow one to two weeks for the entry to appear if the register is held in a different municipality from the one where you filed.

Certificate of residence (住民票) — the household's, obtained by the Japanese spouse. Ask for the version covering everyone at the address (世帯全員) and ask explicitly for 続柄 (relationship to the head of household) and 本籍 (registered domicile) to be included. By law they are left out unless requested, and a single-person copy does not evidence that a couple live together, which is the reason it is being submitted.
